In short
Register in the padrón at Pals town hall as your first step. Then obtain the Catalan health card (TSI) from CatSalut or the CAP Palafrugell. For doctors, go to the CAP Palafrugell (10 km). For emergencies, call 112, or go to Hospital de Palamós (25 km) for urgent care. Private insurance is available for faster access.
Quick facts
- The nearest public hospital is Hospital de Palamós, roughly 25 km from Pals.
- The local primary care centre is the CAP Palafrugell, about 10 km away.
- The health card in Catalonia is called the TSI (Targeta Sanitària Individual), and you must register in the local padrón first.
- Urgent care is at the CAP Palafrugell (daytime) and the Hospital de Palamós emergency department; dial 112 for emergencies.
- Private healthcare is available through private clinics in the region, such as the private wing of Hospital de Palamós and clinics in Girona, without recommending any.
Healthcare is one of the first things to sort out when you move to Pals. The town itself has no health centre or pharmacy, so you will travel to Palafrugell or Palamós for most care. This page explains who can use the public system, how to register, and where to go when you need help.
Public Healthcare and Who Qualifies
Spain's public healthcare system is universal and free at the point of use for residents. In Catalonia, it is run by CatSalut (the Catalan Health Service). As a British person moving to Pals, you are entitled to public healthcare if you are:
- Employed in Spain and paying social security contributions.
- Self-employed and registered with the social security system.
- A UK state pensioner with a valid S1 form (registered with Spain's Instituto Nacional de la Seguridad Social).
- A non-EU retiree who has legally resided in Spain for more than one year and can prove means of support (special agreement).
If you are not yet eligible, you must have private health insurance. UK citizens no longer have automatic access to Spanish state healthcare under the NHS reciprocal agreement (since Brexit), so an S1 form is essential for pensioners. The S1 is issued by the UK's Department for Work and Pensions before you move.
Children under 18 and pregnant women are covered regardless of their parents' employment status, as long as they are registered residents.
Registering: Padrón to Health Card
The first step is registering in the municipal padrón (the residents' register) at Pals town hall (Ajuntament de Pals). This is a legal requirement for anyone living in Spain for more than three months, and it is the key to accessing public services.
You will need:
- A valid passport or EU national ID.
- Your NIE (Número de Identificación de Extranjero) – a foreigner identification number issued by the police or the Spanish consulate.
- Proof of address (a rental contract, property deed, or a recent utility bill in your name).
Once registered, you will receive a padrón certificate. Take this to your nearest CatSalut office or the CAP Palafrugell to apply for your health card – the TSI (Targeta Sanitària Individual). The TSI is a personal card with a health number, and it is what you use to make appointments at the health centre, get prescriptions, and access emergency departments. Processing usually takes a few weeks, so register as soon as you arrive.
You can also apply online through the CatSalut website if you have a digital certificate, but in-person help is often faster for new arrivals.
Your Local Health Centre
Pals does not have its own primary care centre. Residents are assigned to the CAP Palafrugell, about 10 km inland. This is a modern, well-equipped centre with general practitioners, pediatricians, nurses, and a minor injuries unit. To register, you need your TSI card and a completed health centre choice form (available at the centre).
Expect a doctor's appointment by phone or in person within about two days for non-urgent issues. You can request appointments online via the CatSalut app or by calling the centre. Phone lines in Catalan and Spanish – there is no guarantee of English, so it is useful to have a translator or a friend who speaks Spanish.
The CAP Palafrugell is open Monday to Friday (times vary; check the CatSalut website). Outside these hours, urgent care is provided by the Hospital de Palamós emergency department, which is about 25 km away. For after-hours help, you can also call the Catalan medical emergency line, 061, for advice.
Hospitals
For anything beyond primary care, you will be referred to a hospital. The nearest public hospital is Hospital de Palamós, roughly 25 km from Pals. It is a general hospital with a 24-hour emergency department, surgical services, maternity, and outpatient clinics. It is part of the public network, so your TSI card covers your treatment.
More complex cases are referred to Hospital Universitari Doctor Josep Trueta in Girona, about 55 km away. This is a teaching hospital and the main trauma and specialist referral centre for Girona province. Expect a journey of around 50–60 minutes by car; there is no train service between Pals and Girona, so a car is essential for hospital visits.
Hospital Palamós also has a private wing (Hospital Social Palma – not verified) and private consultations, which we discuss below. The public hospital is well regarded, but waiting times for non-urgent specialist appointments can be several months.
Emergencies, Urgent Care and Pharmacies
In an emergency, dial 112 – this is the pan-European emergency number and is free from any phone. For medical emergencies that are not life-threatening, call 061 (Catalan medical emergency line) for advice and potentially a home visit from a doctor.
Urgent care during daytime hours is at the CAP Palafrugell, where the centre has an urgent care (urgències) desk. Outside opening hours, go directly to the Hospital de Palamós emergency department – it is open 24/7. There is no clinical dispatching; you just present yourself at the A&E.
Pharmacies in Spain are easy to spot by their green cross. Pals has no pharmacy, but Palafrugell and Begur both have well-stocked pharmacies. Palafrugell is the larger town, about 10 minutes' drive, and you will find several pharmacies there, including one that usually stays open until late evening on a rota. You will need a prescription from a Spanish doctor for most medicines – the TSI card makes prescriptions significantly cheaper, as the public system covers a percentage of the cost. Keep a stock of any regular medication you take, and bring a doctor's letter in English and Spanish if you carry controlled substances.
Private Healthcare and Insurance
Many British residents living in the area take out private health insurance to avoid waiting lists, to see English-speaking doctors, and to get more flexible appointment times. The private market in Girona is well developed.
Key options include:
- Private insurance from companies such as Sanitas, Adeslas, and Mapfre – most offer English-language policies.
- Private clinics and hospitals in Girona city, including the private wing of Hospital de Palamós, which offers faster access to specialists.
- Using a private GP for primary care; some clinics in Palafrugell and Begur offer private consultations.
Premiums for under-60s typically start around €80–€120 per person per month, depending on coverage and age. You must declare your private cover if you are not yet entitled to public healthcare – otherwise you may face fines. When you do become eligible for the public system, you can keep private insurance as a top-up, or drop it.
Costa Bridge View
Pals is a beautiful town, but its small size means healthcare is not at your doorstep. The 10-minute drive to Palafrugell is manageable, and the 25 km to Palamós is acceptable for most people, though it is a real consideration in an emergency. The public system is good, but waiting times for specialists can be long – private insurance is a sensible extra for many, especially if you have health conditions or prefer English-speaking doctors.
For those planning to work or retire in Pals, the registration steps are straightforward but bureaucratic, and you should allow weeks to get your TSI. If you prefer the convenience of having everything in one village, Pals may frustrate you – but if you have a car and are happy to travel for appointments, it is a perfectly workable base.
