In short
To access public healthcare in Manilva or Puerto de la Duquesa, register on the local padrón, register your S1 or social security entitlement with the INSS, and obtain your tarjeta sanitaria at Centro de Salud San Luis de Sabinillas on Avenida Andalucía. Day-to-day consultations and triage take place at the Sabinillas centre or the consultorio in Manilva village. For scheduled hospital consultations, patients use the public Hospital de Estepona (~20 km), while major emergency and specialist hospital admissions go to Hospital Universitario Costa del Sol in Marbella (~60 km).
Quick facts
- The nearest main public hospital is Hospital Universitario Costa del Sol, a public regional facility roughly 60 km away on the eastern side of Marbella.
- The local public primary care facilities are Centro de Salud San Luis de Sabinillas on Avenida Andalucía and Consultorio Manilva in the village.
- The regional public health card is the tarjeta sanitaria, and registering on the municipal padrón comes first.
- Urgent care is handled locally at the primary health centre, and you should call 112 for all emergencies.
- The nearest major private hospital option is Hospiten Estepona on the coastal A-7, alongside local private facilities like Helicopteros Sanitarios in Los Hidalgos.
Healthcare access is a primary consideration for British retirees, working residents, and second-home owners establishing themselves in Manilva, Puerto de la Duquesa, or San Luis de Sabinillas. The single most important factor to understand is geographic: while day-to-day general practice and basic triage are managed directly within the municipality, major emergency admissions and specialist hospital treatments require travelling up the coast to Estepona or Marbella.
Public Healthcare and Who Qualifies
Public healthcare in Andalusia is managed by the Servicio Andaluz de Salud (SAS). Access is not automatic based on home ownership; it depends on your legal residency and social security status. British nationals qualify for state healthcare under several defined routes:
- UK State Pensioners (S1 Form): Those receiving a UK state pension can obtain an S1 certificate from the NHS Overseas Healthcare Services. Once registered with the Spanish social security authorities, this provides full, reciprocal state healthcare coverage on the same terms as Spanish pensioners.
- Workers and the Self-Employed: Anyone legally employed by a Spanish company or registered as an autónomo (self-employed) pays monthly social security contributions that qualify them and their dependent family members for SAS coverage.
- Convenio Especial: Legal residents who have lived in Spain for at least one full year and do not otherwise qualify for public cover can apply to pay into the Convenio Especial. This is a monthly pay-in scheme providing access to public healthcare without prescription subsidies.
- Temporary Visitors: Non-resident British holidaymakers must carry a valid UK Global Health Insurance Card (GHIC) or European Health Insurance Card (EHIC) for necessary, urgent state treatment during temporary stays.
Registering: Padrón to Health Card
Accessing the public health system requires completing a sequential administrative process. You cannot apply for a health card without establishing local municipal registration first.
- Empadronamiento: Register on the municipal padrón at the Manilva Town Hall (Ayuntamiento de Manilva). You will need proof of address (such as a title deed or rental contract) and your passport or TIE.
- Social Security Registration: Register your entitlement with the Instituto Nacional de la Seguridad Social (INSS), presenting your residency card (TIE), padrón certificate, and either your S1 form or proof of employment.
- SAS Registration: Take the official document issued by the INSS, your padrón certificate, and your TIE to your designated local health centre. Staff will assign you a general practitioner (médico de cabecera) and process your Andalusian health card (tarjeta sanitaria).
Your Local Health Centre
Primary care across the municipality is divided between the coastal community and the inland village:
- Centro de Salud San Luis de Sabinillas: Located on Avenida Andalucía, 3, in San Luis de Sabinillas (telephone: 951 27 06 48). This is the primary clinic serving the coastal strip, including Puerto de la Duquesa. It handles general doctor appointments, nursing care, routine blood tests, and minor ailments.
- Consultorio Manilva: Located in Manilva village (telephone: 952 89 24 51), functioning primarily as a local surgery for residents based in the historic inland centre.
Routine appointments can be booked in advance online via the SAS ClicSalud+ portal, via the regional app, or directly at the administration desk. English is spoken informally by some medical and reception staff, but it is not guaranteed; having functional Spanish or bringing a translator to appointments is advisable.
Hospitals
Manilva does not possess an acute general hospital within its municipal boundaries. Public hospital care is tiered according to medical severity:
- Hospital de Estepona (~20 km): Located on the outskirts of Estepona, this public high-resolution hospital provides scheduled specialist outpatient consultations, diagnostic imaging, and day-case interventions, significantly reducing the need to travel further for routine secondary care.
- Hospital Universitario Costa del Sol (~60 km): Situated on the eastern side of Marbella just off the A-7, this is the main public referral hospital for western Málaga province. It provides comprehensive acute medical services, specialist inpatient wards, maternity care, and major emergency trauma treatment. Journey times typically take 45 to 60 minutes depending on A-7 or AP-7 motorway traffic.
Emergencies, Urgent Care and Pharmacies
For life-threatening emergencies, call 112, the free pan-European emergency number, which provides multilingual operators including English. You can also dial 061 for dedicated Andalusian medical emergency coordination.
For out-of-hours urgent primary care that does not warrant a major trauma centre, the emergency desk (urgencias) at the Sabinillas health centre provides assessment, initial treatment, and ambulance dispatch. If advanced stabilization or emergency surgery is required, patients are transferred directly by ambulance to Hospital Costa del Sol in Marbella.
Local pharmacies (farmacias) are identifiable by a green illuminated cross. In Puerto de la Duquesa, Farmacia la Duquesa is located on Calle Camarón. Outside normal shop hours, pharmacies operate an obligatory rotating on-call system (farmacia de guardia). The current on-duty pharmacy schedule is displayed in the front window of every chemist and listed on municipal notices.
Private Healthcare and Insurance
Because of the distance to the main public hospital in Marbella, many British residents in Manilva and Duquesa maintain private medical insurance. Private policies are also obligatory for non-working visa applicants, such as those on Non-Lucrative or Golden Visas, who require comprehensive policies with no co-payments.
Several private facilities serve the immediate area:
- Hospiten Estepona: Situated on the coastal road near Estepona (~20 km), offering multi-specialist private outpatient clinics, diagnostic facilities, and a 24-hour private emergency room.
- Cenyt Hospital: Located in central Estepona on Avenida de Andalucía, providing private consultations and surgical services.
- Helicopteros Sanitarios: Operates a local private clinic in the Los Hidalgos area near Duquesa, alongside their established membership-based private emergency doctor and home-visit response service.
- Local Private Practices: Independent private general practitioners, such as Doctor González in Plaza Ginebra in Sabinillas, cater to local patients seeking consultations outside the state framework.
Major Spanish and international private insurers (such as Sanitas, DKV, Adeslas, and Bupa) are widely accepted across private facilities in nearby Estepona, Marbella, and Sotogrande.
Costa Bridge View
Healthcare in Manilva and Duquesa works best for residents who are proactive and self-sufficient. Day-to-day general practice at the Sabinillas centre is easily accessible, and basic urgent care is close at hand. However, having your primary public hospital roughly 60 km away in Marbella is a genuine drawback for individuals with complex chronic conditions or those who do not drive. For many British expats, combining the public system with a private insurance policy and reliable private clinic access in Estepona offers the most practical reassurance.
